Property may be in an unlivable condition due to demolition, equipment and work to be performed.

By signing, customer authorizes Blue Waves Restoration to proceed with its recommended procedures to preserve, protect and secure from further damage the property located at the above stated address.

Blue Waves Restoration will bill the insurance company as a courtesy of the property owner. Property owner is ultimately responsible for payment of services rendered by Blue Waves Restoration. Any amount not covered / paid by insurance company within 30 days from the billing date will be billed to the property owner and a lien will be placed on the property for any unpaid balance unless a payment arrangement has been made. Interest will be charged on a balance forward at the monthly rate of 1.5%. Any applicable deductible will be collected at the start of services unless otherwise arranged. Property owner is responsible for collection fees, attorney’s fees and / or court costs in the event that these services become necessary to obtain payment.

In the event that Blue Waves Restoration is not allowed to perform its recommended procedures and / or drying equipment is removed prematurely, I, the customer, agree to release and hold Blue Waves Restoration harmless and indemnify Blue Waves Restoration against any claims or actions that may result from such incomplete procedures, also in the event that the recommended procedures and / or drying equipment is removed prematurely payment in full is due the same day unless otherwise arranged. During the process of dry out, additional damage may occur.

Upon signing customer acknowledges, understands and agree not to hold Blue Waves Restoration responsible for any lost, damaged or stolen property while services are being rendered. By signing, customer also agrees to remove any such items from the damaged property until all services rendered by Blue Waves Restoration have been completed in full.

Customer hereby authorizes and designates Blue Waves Restoration as its representative and grants Blue Waves Restoration the right and ability to negotiate related checks in customer’s name and execute any and all payments received from insurance carrier for services provided herein. Customer agrees and warrants that they shall not negotiate any checks received from insurance carrier for the services herein and such checks shall be held in trust for the direct benefit of Blue Waves Restoration and shall be immediately given to Blue Waves Restoration.

I customer / owner, will request to my insurance that all payments issued out to me should include Blue Waves Restoration name for endorsement or paid directly to Blue Waves Restoration.
